Traci 07-12-2006, 10:12 AM I am not sure. Do you have a case worker at the navy hospital? They would know. I have always done mine with the Navy not a civilian. mossey2000 07-12-2006, 10:29 AM No, Jeff's checking to see if they have an EFM coordinator. The clinic is only accepting active duty at this time. I see civilians because we're 75 miles away. Thanks though! Christy 07-12-2006, 11:41 AM Yes a civilian doctor can fill it out. :yes HEIDI 07-12-2006, 11:46 AM Yep, they sure can!! mossey2000 07-12-2006, 11:47 AM ok thanks! I guess I just need Jeff to get the paperwork for me then. Ste9 07-12-2006, 12:12 PM We just had to renew our EFM also:yes Any dr can fill out the papers which they can mail to you if you call the EFM coordinator. Christy 07-12-2006, 12:18 PM Or you can print out the paper work at http://www.npc.navy.mil under efmp!!!! If you are not near a base you can mail in the paperwork to them without driving all the way there. 1-866-827-5672 for the east coast is the number you can for status verification because the website is always down. ASk for Petty Officer Vass, Chief Harmon, or Mr. Matthews. mossey2000 07-12-2006, 09:22 PM Thanks Christy, that helps a lot! Christy 07-12-2006, 09:49 PM Your very welcome. Just glad that I could help. |
