yes but I am not sure why ......everything that was needed to be entered was.. b/c that why he got delayed an extra day ....of course I have no idea what that form is .....

A "DD 1172" is an application for an ID card. You have to have that to go get an ID without him present. If you Google "DD 1172", you can see what the form looks like.

ahhh.... so i have to write him and get him to get one on base...and then mail it to me ??

I found the DD-1172 online, he has to fill it out and sign it and someone in admin/personnel where he is has to sign it. This was the problem I had. He changed my name in DEERs, I already had an ID and was enrolled in Tricare but couldnt get a new ID with my new last name w/o him signing the DD-1172. So he decided to just come with me to make it easier (DUH)

ok .. I know he didn't change my name b/c we talked about this ...So its ok if I send him the form .... whew.... that helps... I mean I have insurance .... its just I kinda want to get this part over with ...
